Genesis
Vishnu Danta Care began as an outreach initiative of Vishnu Dental College, with an unwavering commitment to providing dental care across all three levels: primary, secondary, and tertiary. The dental school’s state-of-the-art teaching hospital serves as the cornerstone, complemented by a robust portfolio of primary care programs aimed at promoting oral health awareness among the rural population. These programs address the immediate dental needs of patients and refer them to the tertiary care centre for comprehensive and complex treatments. However, a significant number of patients do not follow through on these referrals due to various barriers, including distance, travel expenses, potential loss of daily wages, reliance on family members for transportation, and social inhibitions.
To bridge this gap, the dental school established rural satellite clinics with the objective of making affordable, quality dental care accessible to the rural population. This marked the genesis of the Vishnu Danta Care initiative.
The magnitude of the problem is underscored by the World Health Organization (WHO, 2020) report, which reveals that 95% of adults in India experience dental caries, over 50% suffer from periodontal disease, and oral cancer is among the most common cancers globally. Despite having over 350,000 registered dental surgeons in India—equivalent to approximately 2 dentists per 10,000 people—the majority practice in urban areas, leaving rural regions severely underserved. Common risk factors contributing to the burden of oral diseases include tobacco and alcohol use, limited access to dental care, the prevalence of unqualified practitioners, poor oral hygiene practices, and variations in age and education levels within rural populations. This underscores the timeliness and relevance of the initiative.
In its early stages, the initiative was driven by a small team of committed dentists, with key processes evolving over time. The team studied best practices from institutions such as the Aravind Eye Care System, Madurai, and the LV Prasad Eye Institute to understand the critical success factors of the pyramidal care model. Collaborations with Aravind Eye Care, including on-site visits, provided deeper operational insights. By integrating these learnings with the domain-specific dynamics of dentistry, Vishnu Danta Care evolved into a robust secondary care solution, effectively bridging the gap between tertiary care at Vishnu Dental Hospital and primary care outreach initiatives.
Over the past decade, the model has been rigorously tested for operational viability. It now serves approximately 60,000 patients and performs over 120,000 treatment procedures annually, and has been formalized into a separate entity, Vishnu Danta Care.
Today, Vishnu Danta Care stands as a sustainable social enterprise and a scalable model, aiming to establish 100 rural clinics over the next decade. It is poised to play a significant role in improving access to affordable, quality dental care across rural India.
A Vision for Social Impact
“Being a dental college, we started Rural Satellite Clinics with an intent to provide high quality dental care at a very low-cost to the people in rural and tribal areas. Over time, we also associated with institutions like Aravind Eye Care to improve our efficiency. About 80% of our dental students are women. Many of them stay home after graduation to prepare for post graduate exams or to get married. Due to this gap period, they lose their hand skills. Also, about 30% women dentists quit working after marriage especially when the spouse isn’t a doctor as they generally don’t have competence to set up and run a private practice. Rural clinics provide unique opportunity to women dentists to not only stay in clinical practice but also to gain good expertise on how to manage a private practice”
